Nominate a Worthy Colleague for the Dr. David Bazzo Lifetime Achievement Award

The Dr. David Bazzo CPE Lifetime Achievement Award honors individuals who have made substantial contributions to the field of physician assessment and/or remediation during their career. This award gives public recognition to the individual work and the discipline of physician assessment and remediation.

The CPE Lifetime Achievement Award was named in honor of Dr. David Bazzo in December 2025. 

Individuals eligible for nomination for this award must be or have been active in the field of physician assessment and/or remediation. Award criteria are listed on the nomination form.
